8:o. 68 (of 76, lacks quire c1-8) ll. 35 lines. First page with woodcut illustration (mended in the margins and lower part with some pictorial loss, verso also with some textual loss in lower part).
Later (19th century) vellum, warped covers. 3 leaves mended in lower margin. Some foxing and spotting, some dampstaining in upper margin. Some marginal annotations, upper outer corner with handwritten page numbers.
